Danaparoid Sodium (Orgaran®): An Update on its Approved Indications

Abstract
Background: This review updates the rationale for danaparoid use and the clinical outcomes reported for its approved indications, while identifying areas in which its use may be optimized. Danaparoid, a low-molecular-weight heparinoid, is approved for deep-vein thrombosis prophylaxis and for the treatment of heparin-induced thrombocytopenia (HIT) and disseminated intravascular coagulation (DIC). It consists predominantly of heparan sulphate, of which a small fraction mediates anti-Xa activity through Antithrombin binding, whereas the larger fraction contributes importantly to thrombin generation inhibition and appears to modulate the actions of anti-PF4/polyanion antibodies and attenuate inflammatory responses. These features distinguish danaparoid from other approved antithrombotic agents.
Methods: From an extensive literature search covering 1981 to 2026, new publications, particularly from Japan, have been identified that extend our knowledge of danaparoid.
Results: Despite established dosing recommendations and a substantial body of efficacy and safety data, the clinical value of danaparoid may be underestimated because its pharmacologic behaviour is often misunderstood. This can lead to inappropriate dosing choices. In addition, contraindications and warnings may discourage danaparoid use in clinical settings in which it has shown value. In Japan, the intermittent dosing regimen approved there for DIC has been used unmodified for routine prophylaxis and thrombosis treatment with mixed results. Because of the complex pharmacokinetics of danaparoid, this regimen may be inappropriate in situations where continuous thrombotic and anti-inflammatory protection is required and also for routine thrombosis prophylaxis.
Conclusion: Adopting the established separate dosing recommendations for thrombosis treatment and prophylaxis could improve clinical outcomes. In particular, the therapeutic regime of an intravenous loading bolus followed by continuous therapeutic infusion may better sustain the interactions between danaparoid’s active components and thereby optimise its anticoagulant and immune-modulatory effects.
